In Person

If you are in the state of Illinois and you've accidentally lost or mutilated your driver's license, your only option is to visit a driver services office in person. The steps depend on the circumstances that caused your license to go missing. Let's go over those in more detail:

Stolen Illinois Drivers License

  1. You must visit your city's police department and file a report on your incident. 
  2. After the police report has been created, your next step is to prepare your Identification Documents that you must show (read below for details).
  3. Visit your closest driver services facility.
  4. If you show a valid police report, there is no fee to apply for a duplicate license. Otherwise, you'll need to pay the duplicate license cost.
  5. If you have reason to believe that someone has stolen your identity, please report this immediately to the FTC (Federal Trade Commission). You may download an Identity Theft brochure from their website which will instruct you on what to know, and what to do. 

Lost or Destroyed Drivers License

  1. Plan to visit any driver services facility in the state of Illinois. 
  2. Prepare the Identification Documents that you must show (read below for details).
  3. Bring money to pay the fee for the duplicate license. 

Out of State Drivers

Since you are not in the state of Illinois, you will not be available to show up in person. That's fine, because the Illinois Driver Services will allow you to replace your license in the event that you're away from the state. You have two (2) options. You must either:

  • Order a duplicate/replacement license
  • OR request a temporary driver's license. 

If you will be away from Illinois for more than ninety (90) days, you'll need to order a duplicate. The forms will be sent to your out of state mailing address, and you will receive a license that does not contain a photograph.  However, if you will you are away from the state for less than ninety (90) days, you may request that a temporary permit be sent to you. In order to contact the Illinois DMV, please use any of the following two methods:

  1. Call the License Section at at the following number: (217) 782-2720. OR call the Special License and Re-Examination unit at (217) 782-6901.
  2. If you do not wish to call them, you may send an email. Please compose an online message at the following Illinois Driver Services Contact Page.
    • You'll be asked to enter your name, address, email address, phone number, driver's license number (or SSN), birth date and a brief description of the reason for your request.

Documents Required
