Definition

All applicants for an Arkansas driver’s license or identification card must provide a verifiable Social Security Number (SSN) when making application.  Applicants who are not eligible for a SSN must sign an affidavit stating they do not have a SSN.  The name you request for the driver’s license or ID card must match your name with the Social Security Administration.

First Time Applicants

All new applicants for an Arkansas driver's license or identification card are required to provide proof of legal presence (See Below) in the United States as well as proof of identity (See Below)

Legal Presence

To prove legal presence in the United States one of the following documents must be shown:

• U.S. Passport/U.S. Passport Card
• U.S. Birth Certificate
• U.S. Visa
• Photo Document from DHS -
  (Department of Homeland Security, Bureau of Citizenship and Immigration) -
  (No Border Crosser)
• Photo Military/Military Dependent ID
• Armed Forces Discharge Papers
• Naturalization Certificate 

Proof of Identity

• Two (2) primary documents (See Below), or
• One (1) primary document and one (1) secondary document (See Below).

Additional Requirements For Applicants 18 and Under

• An original or certified copy of a BIRTH CERTIFICATE and any one (1)  secondary document issued by a different source with corresponding identity and/or date of birth.
• SIGNATURE OF PARENT (if living) must be provided on the application  indicating the parent assumes all legal and financial responsibilities associated with the operation of a motor vehicle by the applicant. A step-parent or guardian may sign the application if the individual can present court documents substantiating legal guardian status as provided in ACA §27-16-702.
• SCHOOL ATTENDANCE & GRADE AVERAGE FORMS are mandatory documents required to begin the testing process. These special forms are obtained from the business office at your school. Prior to your application inform your school’s administrative staff of your intentions to test for a driver license and ask them to provide you with proper grade average and attendance forms. The forms are furnished to the school district by the Department of Education. The form must be completed at the school office, signed by a school official, stamped with the school seal, and must be dated no more than 30 days prior to submission of the license application. High school diplomas and GED certificates may also be accepted. Individuals being “home schooled” must provide proof in the form of a notarized copy of the written notice of intent to home school provided by the parent or guardian to the superintendent of the local school district. Adult education program students must furnish proof of enrollment and regular attendance in the program. Report cards are not accepted. Likewise, the reporting forms from the local school districts that appear to have been altered will not be accepted.
• HOME SCHOOL STUDENT FORMS are available through the Arkansas Department of Education, Office of Home Schooling. Telephone contact with the office can be made through             (501) 682-1874      . The reporting forms must be exhibit a legal notary seal when submitted with a license application.

Social Security

Primary Documents

• Photo Driver License (cannot be expired more than thirty-one (31) days)
• Certificate of Birth (US only) 
  Must be original or certified copy, have a raised seal and be issued by the 
  Bureau of Vital Statistics or State Board of Health.
• INS Documents, as follows:
  Certificate of Naturalization (N-550, N-570, or N-578).
  Certificate of Citizenship (N-560, N-561, or N-645).
  Resident Alien Card (I-151, I-551, AR-3, AR-3A, or AR-103).
• Court Order:
  Must contain full name, date of birth and court seal.
  Examples include adoption document, name change document, or gender change document. Does not include abstract of criminal or civil conviction.
• Military ID with photograph.
• Military Dependent ID with photograph.
• Valid Passport. If foreign, appropriate INS document is also required.
• Armed Forces Discharge Papers.

Secondary Documents

• Photo Work/School ID
• Vehicle Registration/Title
• Bureau of Indian Affairs Card/Indian Treaty Card
  (Tribal ID Card is NOT acceptable)
• Court Order (date of birth not present)
• Health Insurance Card
  (Blue Cross/Blue Shield, Kaiser, HMO, etc. which has been in effect at least five (5) years.)
• IRS/State Tax Forms (W-2 NOT acceptable)
• Marriage Certificate/License
• Medical Records (from Doctor/Hospital)
• Parent/Guardian Affidavit
  Parent/guardian must appear in person, prove his/her identity and submit a certified/notarized affidavit regarding the child's identity. Applies only to minors.
• Concealed Handgun License
• Pilot's License
• Certified School Record/Transcript
