In Person

To replace a driver’s license or identification card that has been lost, stolen, or mutilated,

  • Please take one primary (See Below) and one secondary identification document (See Below) or three secondary identification documents (See Below) to your local Office of Motor Vehicles. A complete list of acceptable identification documents is in Section 1, Policy 6.00.
  • Proof of insurance is required for issuance of the duplicate driver's license if you own an automobile.
  • Minors, 17 years of age or younger, must bring their certified birth certificate, one secondary identification document, and be accompanied by custodial parent or legal guardian with picture I.D. In case of joint custody, only domiciliary parent may sign.
  • We do not process this transaction online.

$16.00 (This fee is an estimate as an additional parish fee may exist in some offices which will not to exceed $3) CASH or Money Order. The charges for a duplicate driver's license will be waived if the license has been stolen and a police report is submitted indicating such. Documents Needed

DEFINITION

All applicants for drivers' licenses or identification cards are required to present proof of identification as defined herein. 

FIRST TIME APPLICANTS

Applicants will be required to provide one of the following combinations:

  • One primary and two secondary documents OR
  • Two primary documents OR
  • If applicant has a Louisiana identification card/driver's license with a photo which clearly identifies the applicant and the Social Security Number is part of the file, additional identification is NOT required.

ADDITIONAL REQUIREMENTS FOR APPLICANTS SEVENTEEN AND UNDER

  • The signature of the custodial parent or legal guardian is required for the issuance of any license (including first time applications, duplicates and changing restrictions).
  • Documentation must be presented to prove custody or legal guardianship.
  • Identification must be presented by the minor and the parent or guardian.
  • Only the domiciliary parent may sign if joint custody has been awarded.

SOCIAL SECURITY REQUIREMENT

All applicants eligible for a social security number must provide the assigned number when applying for a driver's license. Although the Social Security Number will not be displayed on the face of the license, that number must be entered in the department's internal records. 

In order to comply with the Military Selective Service Act, R.S. 32:40:1321(D)(1), the Social Security Number is required from all male US citizens or immigrants age 15 to 26 who apply for a driver's license or ID card. Failure to provide the SSN will result in the denial of issuance.

OUT-OF-STATE TRANSFERS

Applicants must provide all of the following:

  1. Out-of-state license
  2. One primary document
  3. Social Security Number

EXCEPTIONS ON OUT-OF-STATE TRANSFERS

  • If the applicant has had a previous Louisiana license or identification card and it is currently on photo retrieval and the applicant is clearly identified, then the above requirements may be waived.
  • If the out-of-state photo license has been lost, an official driving record from the last issuance state must be provided.

ELDERLY APPLICANTS

Every effort should be made to accommodate the elderly. 

EXCLUSIVE LIST OF ACCEPTABLE PRIMARY & SECONDARY IDENTIFICATION DOCUMENTS.

PRIMARY DOCUMENTS

A certified copy of birth certificate (long form), original certificate of birth (does not have to have seal), birth registration card or certificate issued by a state or county bureau of vital statistics with raised agency seal, birth card (short form).

Passport book.
  • Foreign passports must be appropriately stamped and accompanied by proper immigration documents (I-94). See Policy 6.02, Section 1 for procedures on Aliens.
  • Applicants 17 and under must also present a certified birth certificate or documentation proving custody/legal guardianship in addition to the passport.

Passport Card.
Permanent Resident Card (I-551) Note: If the photograph on the I-551 was taken prior to the applicant's 14th birthday and the applicant is now over age 14, the applicant must be referred to the Department of Homeland Security for issuance of a new I-551 prior to processing the application.

Certificate of naturalization: NOTE--This document grants U.S. Citizenship. It is a federal crime to duplicate this form.
  • Do not accept copies of this document.
f.
Certification of birth abroad issued by the United States Department of State.
g.
Native American tribal document.
h.
The Department of Homeland Security Refugee travel document (I-571).
i.
The Department of Homeland Security Permit to reenter the United States (I327).

SECONDARY DOCUMENTS

1. Out-of-state driver's license with a photo which clearly identifies the individual
  • An official copy of driving record from last state of issuance will be required when the applicant has lost his out-of-state license.
  • Any foreign driver's license must be accompanied by proper immigration documents.
2. Social Security card or official written documentation verifying the Social Security Number from the Social Security Administration or specifying that no number has been assigned or that the applicant is ineligible for an SSN.
3. Out-of-state photo identification card issued by a state motor vehicle department.
4. Student I.D. card for a Louisiana college or university when accompanied by a 100% fee paid receipt for the current semester.
5. Original documents of adoption papers.
6. Original of a high school, college or university diploma.
7. Original marriage license or certificate of marriage issued by a county, parish or city in the U. S. or an original or certified copy of a divorce from a county, parish or city in the U. S.
8. Church Baptismal Certificate.
9. Louisiana voter registration card.
10. Official certified deeds or title to property in Louisiana, including burial plot deeds.
11. Vehicle registration or certificate of title of vehicle in applicant's name.
12. Local utility statements showing name and address of the individual applying or receipt showing public utilities have been turned on (Do not accept these statements that are from a city other than the application city).
13. Insurance policy (health, home, life, auto).
14. One payroll stub (Cannot be handwritten).
15. Temporary resident card (Form I-688).
16. High school year book - provided the picture clearly identifies applicant as the same.
17. Medicare/Medicaid card or medical eligibility card.
18. Prison release documents or letter from probation officer [denote a PEN number (SID) on fly sheet whenever possible].
19. Military discharge papers (DD214).
20. Selective Service notification to applicant at his address.
21. Original or certified copy of any professional degree, certificate or license.
22. School records OR at least two report cards from separate years.
23. Motor vehicle lien instrument.
24. Any government law enforcement officer's identification or badge, if in uniform, a police unit, etc. (does not include private security company guards).
25. Letter of verification/introduction from another state agency responsible for placement of deprived or handicapped persons, (i.e. Blind Services)
26. Employment ID with photo provided the photo clearly identifies applicant as same. USE DISCRETION. (Accept ID's from major corporations, hospitals, governmental agencies, etc.)
27. Identification card issued by federal, state, or local government agency or entity provided it contains a photo which clearly identifies the individual and information such as name, date of birth, sex, height, and address.
28. W-2 forms for two years.
29. Medical Card must be presented for the issuance of duplicate CDL license.
30. Driver's log book may be used as a secondary document on a CDL license.
31. Certificate from the Department of Education for driver's education course completion.
32. Six-Hour Pre-licensing Completion Certification.
33. Completion Certificate from the 38 hour Pre-licensing course.
34. Original "Refugee Resettlement Program" letter with color picture of applicant--the color picture will have the agency director's signature across it and a raised agency seal overlapping. This letter will be valid for 15 days from the issue date.
35. Louisiana Department of Public Safety & Corrections prison or parish I.D. card containing photograph, name, race, sex, and date of birth.
36. Valid U.S. military identification card or draft record or military dependent identification card.
37. TWIC (Transportation Worker Identification Credential) card.

Current U.S. Coast Guard Merchant Mariner card.
Employment/work authorization document issued by the Department of Homeland Security (I-688B or I-766).
