If your driver license, permit or ID card is lost, destroyed or becomes difficult to read, you may get a duplicate online or by applying in person at a DMV office. You may visit any office within the state regardless of your city or county of residence. See Fees.
Online
You may order a duplicate of a non-commercial license, instruction permit or ID card online. You will receive a new card in the mail with your existing photo. Sorry, no address changes, name changes, motorcycles or commercial licenses or permits. These must be done in person.

In Person
If you apply for your duplicate in person at a DMV office, it is best to bring as much identification documentation as possible. This will help confirm your identity and speed up the process. The Department may use your existing record and photograph currently on file when necessary.
Nevada uses a central issuance licensing system. You will be given a paper interim document and your permanent license or ID will be mailed to you.
Out-of-State
Nevada residents out-of-state may also obtain a duplicate by submitting an Application for Nevada Driver License By Mail (DMV-204). Some duplicates issued by mail may not have your photo. You are required to surrender this "valid without photo" document for a photo license within 24 days of returning to Nevada.

Documents must be valid originals or certified copies that were issued in the United States (except for foreign passports). We do not accept photocopies. Permanent Resident Cards must be updated at age 14.
Maiden Names/Name Changes
You must present documentation of any name change(s) if you will be presenting a proof of identity document that does not contain your current legal name. This may include a Marriage Certificate, divorce decree, adoption records or court order. In a case of multiple name changes, you may be asked for records of each change.
If you lack these documents, contact your state or county Vital Statistics Office, www.vitalchek.com or a similar service and/or the Social Security Administration.

Homeless Exemption
The Department is authorized to waive the $17 fee for homeless persons to obtain a duplicate Nevada driver's license or identification card. This is a one-time exemption for duplicates only. You must apply for this in person at a DMV office and submit the proper proof of identity and forms.
We suggest that you pay the $3 photo fee at the time you receive your duplicate if possible. You may be required to reimburse this fee to the Department at the time of your next regular renewal, if you are employed at the time of the renewal.
