LEARNERS PERMIT 15 YEARS OLD

Please take

  • Your certified birth certificate.
  • Social Security Card.
  • Certificate of successful completion of a 38 Hour Drivers Education Course approved by the Department of Public Safety & Correction or the Department of Education to a local Office of Motor Vehicles
  • If you do not have the original Social Security Card, you will need to provide an additional secondary document (See Below). Your parent or legal guardian must accompany you with picture I.D. In case of joint custody, only domiciliary parent may sign. If a minor is accompanied by a mother who has since re-married, the mother must present documentation to show the connection in the names, such as a certified copy of her divorce decree and new marriage license.
  • You will be required to take and pass the written and vision tests.
  • Fee: estimated $24.50 (a parish fee may exist which will not to exceed $3) CASH
  • A 15 year old cannot get an Intermediate license until he/she is 16 years old and has had the Learners Permit for a minimum of 180 days.
  • This license will enable the minor to drive while being accompanied by a licensed parent, guardian or licensed adult at least age twenty-one or older.

  • If you do not have the original Social Security Card, you will need to provide an additional secondary document. Your parent or legal guardian must accompany you with picture I. D. In case of joint custody, only domiciliary parent may sign. If a minor is accompanied by a mother who has since re-married, the mother must present documentation to show the connection in the names, such as a certified copy of her divorce decree and new marriage license.
  • You will be required to take and pass the written and vision tests.

  • This license authorizes the holder to drive while being accompanied by a licensed driver. The restriction may be removed immediately upon the applicant successfully passing the on-the-road driving examination.

INTERMEDIATE LICENSE 16 YEARS OLD

After completing the learners license/permit stage and upon reaching at least sixteen years of age, a minor may apply for a Class "E" Intermediate License. This license must be maintained for a minimum of one year from the date of issuance or until the driver has reached the age of seventeen.

To have the Intermediate license issued, please take

  • Your Learner’s Permit.
  • Certified birth certificate.
  • Proof of insurance for the automobile to a local Office of Motor Vehicles
  • Your parent or legal guardian must accompany you with picture I.D and provide a written statement attesting that you have a minimum of fifty (50) hours of behind the wheel driving experience with a licensed parent, guardian, or adult at least twenty-one (21) or older. At least fifteen (15) of these hours must be night time driving. In case of joint custody, only domiciliary parent may sign. If a minor is accompanied by a mother who has since re-married, the mother must present documentation to show the connection in the names, such as a certified copy of her divorce decree and new marriage license.
  • The vehicle must have a current safety inspection sticker, current license plate validation sticker, and current certificate of registration.
  • You will be required to take and pass the driving skills and vision tests. Fee: $16.00 (Fees may vary at the local office) CASH


NOTE: Driving skills tests are NOT given between 11:00 AM and 1:00 PM. We suggest you arrive no later than 3:00 PM. Skills test are NOT given during inclement weather.

You have the option of going to a state certified Third Party Tester to be administered a driving skills test. They can charge up to $30.00 for the driving skills test. Third Party Tester list. You must report to the Third Party Tester with Test History Form or a valid learner's permit issued by the Office of Motor Vehicles. A Third Party Tester can lease a vehicle to you to take the driving skills test. They also can administer the test in inclement weather and at your convenience, usually anytime of the day or night.

This license allows the minor to drive alone or with other passengers in the vehicle, but restricts him from driving between the hours of 11:00 p.m. to 5:00 AM unless he is accompanied by a licensed parent, guardian, or adult at least twenty-one or older. After the one year period or once the driver reaches age seventeen, the minor may qualify for a permanent Class "E" license.

1. Out-of-state driver's license with a photo which clearly identifies the individual
  • An official copy of driving record from last state of issuance will be required when the applicant has lost his out-of-state license.
  • Any foreign driver's license must be accompanied by proper immigration documents.
2. Social Security card or official written documentation verifying the Social Security Number from the Social Security Administration or specifying that no number has been assigned or that the applicant is ineligible for an SSN.
3. Out-of-state photo identification card issued by a state motor vehicle department.
4. Student I.D. card for a Louisiana college or university when accompanied by a 100% fee paid receipt for the current semester.
5. Original documents of adoption papers.
6. Original of a high school, college or university diploma.
7. Original marriage license or certificate of marriage issued by a county, parish or city in the U. S. or an original or certified copy of a divorce from a county, parish or city in the U. S.
8. Church Baptismal Certificate.
9. Louisiana voter registration card.
10. Official certified deeds or title to property in Louisiana, including burial plot deeds.
11. Vehicle registration or certificate of title of vehicle in applicant's name.
12. Local utility statements showing name and address of the individual applying or receipt showing public utilities have been turned on (Do not accept these statements that are from a city other than the application city).
13. Insurance policy (health, home, life, auto).
14. One payroll stub (Cannot be handwritten).
15. Temporary resident card (Form I-688).
16. High school year book - provided the picture clearly identifies applicant as the same.
17. Medicare/Medicaid card or medical eligibility card.
18. Prison release documents or letter from probation officer [denote a PEN number (SID) on fly sheet whenever possible].
19. Military discharge papers (DD214).
20. Selective Service notification to applicant at his address.
21. Original or certified copy of any professional degree, certificate or license.
22. School records OR at least two report cards from separate years.
23. Motor vehicle lien instrument.
24. Any government law enforcement officer's identification or badge, if in uniform, a police unit, etc. (does not include private security company guards).
25. Letter of verification/introduction from another state agency responsible for placement of deprived or handicapped persons, (i.e. Blind Services)
26. Employment ID with photo provided the photo clearly identifies applicant as same. USE DISCRETION. (Accept ID's from major corporations, hospitals, governmental agencies, etc.)
27. Identification card issued by federal, state, or local government agency or entity provided it contains a photo which clearly identifies the individual and information such as name, date of birth, sex, height, and address.
28. W-2 forms for two years.
29. Medical Card must be presented for the issuance of duplicate CDL license.
30. Driver's log book may be used as a secondary document on a CDL license.
31. Certificate from the Department of Education for driver's education course completion.
32. Six-Hour Pre-licensing Completion Certification.
33. Completion Certificate from the 38 hour Pre-licensing course.
34. Original "Refugee Resettlement Program" letter with color picture of applicant--the color picture will have the agency director's signature across it and a raised agency seal overlapping. This letter will be valid for 15 days from the issue date.
35. Louisiana Department of Public Safety & Corrections prison or parish I.D. card containing photograph, name, race, sex, and date of birth.
36. Valid U.S. military identification card or draft record or military dependent identification card.
37. TWIC (Transportation Worker Identification Credential) card.
