Duplicate driver license, instruction permit or ID card
A duplicate license, instruction permit or ID card is required when:

You change your name (you will need to provide acceptable documents for proof of name change).

Your license, permit or ID card is lost or stolen. If you are a Wisconsin resident and you are temporarily out-of-state, you can obtain a duplicate license.

You have changed gender, (you will need to show proof by court order or physician's report).

Your date of birth is incorrect (requires proof of date of birth. If birth certificate, it must be a certified copy, not hospital certificate).

You have a commercial driver license and your address has changed.
You may apply for a duplicate driver license or ID card online if there are no changes needed other than an address correction. (Duplicate instruction permits and commercial driver licenses are not available online.) If there are changes necessary other than an address correction, you will need to visit a DMV service center to apply for a duplicate driver license, instruction permit or ID card. DMV customer service centers accept cash or checks only. Credit and debit cards are accepted for online transactions. You will need to pay the fee for a duplicate license, permit or identification (ID) card. If your license, permit or ID card has or is about to expire, you must renew it.
If you visit a DMV service center, you will need to complete form MV3001
- Wisconsin Driver License (DL)/Identification Card (ID) Application, and show proof of identity. You must also know your social security number. It is recommended that you bring your social security card with you at the time of application. If you are not a U.S. citizen, you will need to provide documentary proof of your lawful permanent or conditional resident or temporary visitor status.
If you are under 18 years of age and applying for a duplicate license or instruction permit, you must visit a DMV service center. Sponsorship by a parent or legal guardian is required. If you are under 18 years of age and want to obtain a duplicate instruction permit, you will also need to provide a driver education course completion certificate or proof of current enrollment in the behind-the-wheel portion of an approved driver education course.

Lost or stolen driver license, identification card
If your license or identification card was recently lost or stolen, you can apply for a duplicate either online or at any Division of Motor Vehicles (DMV) service center (excluding renewal-only offices). Out of state addresses or special mailing addresses are not allowed via the online application.
If your license was lost or stolen, and you are temporarily out of state, you can replace the license through the mail using your temporary out of state address.
Identity theft
If you have reason to believe your identity has been stolen, contact your local law enforcement agency immediately.
The Wisconsin Office of Privacy Protection is a resource center for individuals, businesses, law enforcement and government on issues of privacy protection and identity theft.
You can also ask the DMV to place a notation on your driver record so that DMV and law enforcement will require additional identification documents when you conduct business with them. The statement will be “verify identification documents,” and it will require anyone (including you) attempting to use your identity to provide the following items:

Two forms of identification listing your name with either your photograph or signature.
